The deadline for Municipality Modernization Form submissions has been changed from November 15, 2025, to December 1, 2025. The FY21 and FY22 base calculation form can be found here. Once completed, the form should be emailed to the sales tax department for processing. The FY25 actual expenditures should be submitted through TAP.
