NOTIFICATION PROCEDURE

Please notify the Mississippi Department of Revenue if you have filed bankruptcy. Notification provided to the DOR Bankruptcy Section must include the following information:

• Taxpayer Name(s)
• Tax Account Number(s)
• Bankruptcy Case Number
• Bankruptcy Chapter
• Specific Court District in which the Case is Filed

This information should be submitted in writing to P.O. Box 22808, Jackson, MS 39225-2808 or faxed to 601-923-7334.
